This site will help guide you through the process of becoming an employee or volunteer at Mt. Gilead Camp.
On this site is your Onboarding Checklist, Links and Instructions to the required Background Clearances, Government Required Forms. If you are under 18 years of age, you will find two additional documents. The only document not listed on this site is your Staff Agreement. Your staff agreement will be emailed to you from employment@mtgileadcamp.org and needs to be signed from a tablet or phone. If you are a visual person and would like a physical checklist to keep track of your progress, you can print one here.
STEP 1 – Your Checklist (Pick Your Column)
First-Time Staff Checklist
First-time staff will need to do the following 8 Steps. (Instructions for these are on this page)

ARE YOU UNDER 18?
8. Get your Workers Permit. If You Live In PA, Get Your Workers Permit At School. It’s best to do this before school ends. If you DO NOT live PA, we will take you to get your worker’s permit in PA when you arrive at camp. You will need to fill out the application and get it notarized before coming to camp (their rule, not ours).
9. Have Your Parent Sign The Authorization To Ride From (optional)
MAIL THE FOLLOWING TO CAMP (at one time):
10. Mail Your Two Printed Pa Clearances, Printed FBI Fingerprint Results, Your Filled Out W4, I9 & Res. Cert. Form, Your I9 ID photocopies. If you are under 18, your (optional) Authorization To Ride Form signed by your parent. If you live in PA, you will bring your worker’s permit to camp with you. If you live outside of PA, you will mail the pre-filled & notarized application to camp with your other documents.

Returning Staff Checklist

If your clearances were done more than five years ago, you will need to renew these this year. If you have worked for Mt. Gilead Camp for less than three years, you will not need to renew your clearances this year.

4. Get your FBI Fingerprint Clearance Completed In Your State by May 1st.

If your ID documents used in last year’s I9 have not expired or changed (you must check), then you do not need to do the I9 again. If you have not changed your home address (changed license etc.) then you do not need to do the Res. Cert. Form again.

STEP 2 – Your Clearances
2 out of the 3 required clearances you can do online anytime and will need to print the results at home & mail the results to camp: These two online clearances are:
1. PA Criminal History Clearance (EPatch) See Below
2. PA Child Abuse Clearance (Compass) See Below
3. FBI Fingerprint Background Clearance (Identogo). See Below
Below are the instructions to do these three tasks as soon as you are able.
Employee clearances last 5 years in the state of Pennsylvania. If your latest clearances were renewed less than five years ago, you do not need to do the clearances this year.
If you have volunteered at Mt. Gilead Camp before (or at your church), and are now working at camp for the first time, you must re-do your clearances as an employee instead of a volunteer. *There will be a charge involved for each new clearance*.
*Warning*: This will be the most difficult clearance process because you are required to create a login account with the PA Compass Website, are assigned a temporary password, then emailed a link, then will be required to change your password again after logging in!
How to get the PA Child Abuse History Clearance:
Visit: https://www.compass.state.pa.us/CWIS/Public/Home/CreateNewAccount
Click NEXT.
Fill out the Keystone ID Profile Form
You will receive a confirmation email from the Keystone Website
With your new password written down, visit
https://www.compass.state.pa.us/CWIS/Public/AccountType to login for the first time.
Click ACCESS MY CLEARANCES
Log in. Remember, Passwords are case sensitive. Create New Password,
Log in Again
Complete the Application Online
How to get the PA Criminal History Clearance:
Visit: https://epatch.state.pa.us/
Staff: Click “Submit A New Record Check” (Grey Button)
Volunteers: Click “New Record Check” (Yellow Button)
Select “Accept” at the bottom (if asked)
Select “Individual Request”
Fill out the form and submit information and pay.
If you are out of state (not living in PA) then you need to wait until you are at camp in order to get your fingerprinting done. We are able to assist in transportation if you do not have a vehicle at camp.
How to get the FBI Fingerprint Background in PA:
For Staff & Volunteers Visit: https://www.identogo.com/services/live-scan-fingerprinting
Select Your State (PA)
When asked. select “Digital Fingerprinting” as the service you need.
Fill in the required employer fields exactly as shown below:
Service Code (if asked): 1KG756
A. Employer Name: Mt Gilead Camp & Conference Center
B. Address: 440 E. Rinker Rd Stroudsburg PA 18360
Click Next
C. Fill in the required citizenship field
D. In the “Personal Questions” tab, select “no” to authorization/coupon code
E. Fill out all physical appearance information
F. In the “Mailing Address” tab, enter your (license or home) mailing address (Not your college address)
G. Under document section, select the document you will have with you when at camp (i.e. drivers license or passport), make sure your name in Section B matches the name on this ID exactly
H. Under the location tab, use the zip code you will be in to take the text (college vs home)
I. Take the email you receive from identoGo (or your states agency), print it, and mail it in with your other documents.
J. You will need to complete your physical fingerprinting by May 1st in order to receive the results before beginning employment at Mt. Gilead Camp.

STEP 3 – Your W4, I9 & Residency Certification Form
GETTING YOUR GOVERNMENT FORMS
The Government Requires 3 Forms when you start employment.
1. W4. This will tell the IRS how much tax to take from your weekly paycheck.
2. I9. This form is an employment eligibility form required by the Department of Immigration.
3. Residency Certification Form (RCF). The state of PA needs to know where you live for your Local Earned Income Tax
B) Fill out the form. For younger staff, your parents may be able to help you fill out this W4. Anyone may also look into this article for assistance.
C) Make sure to mail your completed W4 with your other required documents to Mt. Gilead Camp
A) Download the I9 Form Here and Print
B) Fill out the form. Since your I9 verifies that you are a legal citizen, it also requires that we have legal documents on file at camp that prove you live in the US. Along with your I9 we will need a photocopy of your passport mailed to us. If you do not have a passport, you will need a combination of other documents (photocopied). The most common are listed below (you must have 1 blue and 1 pink document, you cannot have two of the same color:
Option1: Just Your Passport (Best Option)
Option 2: Drivers License & Social Security Card
Option 3: Drivers License & Birth Certificate
Option 4: School ID Card & Social Security Card
Option 5: School ID Card & Birth Certificate
Under 18 years old and don’t have the above documents? You can use:
Option 6: School Report Card & Social Security Card (or) Birth Certificate
Option 7: Doctor or Hospital Record & Social Security Card (or) Birth Certificate
C) Use the I9 Form to see other documents you can use for ID. Do not mail your paperwork to camp without the required photocopies of your ID’s included with your I9.
B) Fill out the form. Your Residency Certification Form tells the Government where you live for tax purposes. When filling out this form, please use your home address, not your school address (if you are at college). (i.e. what’s listed on your license as your address.) Do not use the summer camp address as your address for this form.
STEP 4 – Are you under 18?
If you are under 18 years old you need a worker’s permit to work in the state of Pennsylvania. If you live in PA already, you can get your worker’s permit at school. If you live outside of Pennsylvania, we will take you to get your worker’s permit once you arrive at camp. To prepare for this, you will need to get the application filled out and notarized, then mailed to camp ahead of time. In order to travel in another staff member’s car outside of camp (your time-off), we need permission from your parent allowing you to do this (it’s an insurance thing). For this reason, we have an Authorization To Ride Form that they will need to sing.
1. WORKERS PERMIT
If you live in Pennsylvania and are under 18 years old, you can get your worker’s permit at school. Start with contacting your guidance counselors if you are unsure of the process. You will need your parent to go with you to the school, and you will need your birth certificate at the time. Do not mail your workers permit to camp, but bring it with you on your first day.
FOR THOSE LIVING OUTSIDE OF PA UNDER 18 YEARS OLD
If you are under 18 years old and live outside of Pennsylvania, do not get a worker’s permit at your school, Mt. Gilead Camp will take you to get your worker’s permit when you arrive at camp. However, you will need to bring the application to camp filled out and notarized.
A) Download & Print the Pocono Mountain Workers Permit Here
B) You will need to fill out the second half of page 2 with your parent – do not sign the form yet.
C) After completing the information, you will need to get the document notarized locally in your town. Your parent will sign the document in front of the notary.
D) Make Sure To Mail the Notarized Document To Camp With Your Other Documents
2. CPS DISCLOSURE STATEMENT
If you are under 18 years old, you are not required to do the FBI Fingerprinting. However, Child Protection Services does require a signed disclosure for all employees working with children under the age of 18.
A) Download & Print The CPS Disclosure Form Here
B) Mail This Form In With Your Other Documents (All At One Time)
3. AUTHORIZATION TO RIDE (OPTIONAL)
If you are under 18 years old and would like to travel out of camp with other staff members in their personal cars, your parent will need to sign the Authorization To Ride Form.

B) Mail This Form In With Your Other Documents (All At One Time)
STEP 5 – Mail all required documents at one time
Please mail all documents to Mt. Gilead Camp at one time and not individually as you get them accomplished.
